FDC Assault Advisories | May 1 – May 14, 2018
This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty.
CROSS CITY, Fla. – On May 1, an inmate assaulted a correctional officer at Cross City Correctional Institution.
Inmate George Clark (120876) assaulted the officer at approximately 10:05 p.m. Inmate Clark struck the officer in the face. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Clark will receive a disciplinary report for this assault.
PUNTA GORDA, Fla. – On May 2, an inmate assaulted a correctional officer at Charlotte Correctional Institution.
Inmate Joshua Baker (E32461) assaulted the officer at approximately 1:10 p.m. Inmate Baker struck the officer in the face. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Baker will receive a disciplinary report for this assault.
LIVE OAK, Fla. – On May 2, an inmate assaulted a correctional officer at Suwannee Correctional Institution.
Inmate Bryan Thomas (H15724) assaulted the officer at approximately 5:50 p.m. Inmate Thomas bit and kicked the officer in the face. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Thomas will receive a disciplinary report for this assault.
RAIFORD, Fla. – On May 3, an inmate assaulted a correctional officer at Florida State Prison.
Inmate Kenneth Hooker (I09088) assaulted the officer at approximately 6:28 a.m. Inmate Hooker head-butted the officer. Staff responded appropriately, and the inmate was subdued.
Inmate Hooker will receive a disciplinary report for this assault.
PUNTA GORDA, Fla. – On May 3, an inmate assaulted correctional officers at Charlotte Correctional Institution.
Inmate Joseph Debell (M44619) assaulted the officers at approximately 11:15 a.m. Inmate Debell struck one officer in the head and another in the face and chest. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officers and noted injuries. Inmate Debell will receive a disciplinary report for this assault.
PUNTA GORDA, Fla. – On May 3, an inmate assaulted a correctional officer at Charlotte Correctional Institution.
Inmate Kristopher Hammerich (165340) assaulted the officer at approximately 8:45 p.m. Inmate Hammerich scratched the officer’s arm. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Hammerich will receive a disciplinary report for this assault.
MILTON, Fla. – On May 4, an inmate assaulted a correctional officer at Santa Rosa Correctional Institution.
Inmate Carlos Figueroa (X86053) assaulted the officer at approximately 6:18 a.m. Inmate Figueroa punched the officer in the lower body. ORLANDO, Fla. – On May 4, an inmate assaulted a correctional officer at the Central Florida Reception Center.
Inmate John Sikorsky (K75561) assaulted the officer at approximately 8:40 p.m. Inmate Sikorsky struck the officer in the chest. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Sikorsky will receive a disciplinary report for this assault.
PERRY, Fla. – On May 5, an inmate assaulted correctional officers at Taylor Annex.
Inmate Thaddeus Martin (M85852) assaulted the officers at approximately 12:15 a.m. Inmate Martin struck the officers repeatedly. Staff responded appropriately, and the inmate was subdued.
Inmate Martin will receive a disciplinary report for this assault.
LAKE CITY, Fla. – On May 5, an inmate assaulted correctional officers at Columbia Correctional Institution.
Inmate Corey Fulbright (C09886) assaulted the officers at approximately 11:40 a.m. Inmate Fulbright elbowed an officer in the chest, struck another in the stomach, and struck a third in the head. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officers and noted injuries. Inmate Fulbright will receive a disciplinary report for this assault.
RAIFORD, Fla. – On May 5, an inmate assaulted a correctional officer at Florida State Prison.
Inmate Patrick Deboard (L25900) assaulted the officer at approximately 8:32 p.m. Inmate Deboard head-butted the officer. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Deboard will receive a disciplinary report for this assault.
LAKE CITY, Fla. – On May 6, an inmate assaulted a correctional officer at Columbia Annex.
Inmate Dayan Williams (W45466) assaulted the officer at approximately 10:39 a.m. Inmate Williams head-butted the officer in the face. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Williams will receive a disciplinary report for this assault.
MILTON, Fla. – On May 7, an inmate assaulted a correctional officer at Santa Rosa Correctional Institution.
Inmate Samuel Staten (C04728) assaulted the officer at approximately 10:41 a.m. Inmate Staten head-butted the officer in the face. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Staten will receive a disciplinary report for this assault.
OCALA, Fla. – On May 7, an inmate assaulted a correctional officer at Lowell Annex.
Inmate Unique Edwards (K89201) assaulted the officer at approximately 7:32 p.m. Inmate Edwards kicked the officer in the knee. Staff responded appropriately, and the inmate was subdued.
Inmate Edwards will receive a disciplinary report for this assault.
DE FUNIAK SPRINGS, Fla. – On May 8, an inmate assaulted a correctional officer at Walton Correctional Institution.
Inmate Kerween Louinord (M51684) assaulted the officer at approximately 12:05 p.m. Inmate Louinord kicked the officer in the leg. Staff responded appropriately, and the inmate was subdued.
Inmate Louinord will receive a disciplinary report for this assault.
MILTON, Fla. – On May 8, an inmate assaulted a correctional officer at Santa Rosa Correctional Institution.
Inmate Gregory Moreland (L91755) assaulted the officer at approximately 9:39 p.m. Inmate Moreland kicked the officer in the leg several times. Staff responded appropriately, and the inmate was subdued.
Inmate Moreland will receive a disciplinary report for this assault.
PUNTA GORDA, Fla. – On May 9, an inmate assaulted a correctional officer at Charlotte Correctional Institution.
Inmate Adam Burns (C03947) assaulted the officer at approximately 9:30 p.m. Inmate Burns struck the officer in the face. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Burns will receive a disciplinary report for this assault.
CROSS CITY, Fla. – On May 10, an inmate assaulted correctional officers at Cross City Correctional Institution.
Inmate Sergio Henderson (V18889) assaulted the officers at approximately 10:11 a.m. Inmate struck one officer in the face and stabbed another in the leg with a makeshift weapon. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Henderson will receive a disciplinary report for this assault.
DE FUNIAK SPRINGS, Fla. – On May 10, an inmate assaulted a correctional officer at Walton Correctional Institution.
Inmate Oliver Mora (M26835) assaulted the officer at approximately 5:40 p.m. Inmate Mora bit the officer’s arm. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Mora will receive a disciplinary report for this assault.
TRENTON, Fla. – On May 10, an inmate assaulted a correctional officer at Lancaster Correctional Institution.
Inmate Dontavious Powell (Y50020) assaulted the officer at approximately 9:10 p.m. Inmate Powell struck the officer in the shoulder. Staff responded appropriately, and the inmate was subdued.
Inmate Powell will receive a disciplinary report for this assault.
OCALA, Fla. – On May 10, an inmate assaulted a correctional officer at Marion Correctional Institution.
Inmate Yordy Cambon (M88975) assaulted the officer at approximately 10:56 p.m. Inmate Cambon struck the officer in the face. Staff responded appropriately, and the inmate was subdued.
Inmate Cambon will receive a disciplinary report for this assault.

Inmate Quantez Ealy (C03113) assaulted the officer at approximately 9:22 a.m. Inmate Ealy struck the officer in the face. Staff responded appropriately, and the inmate was subdued.

JASPER, Fla. – On May 11, an inmate assaulted a correctional officer at Hamilton Annex.
Inmate Jospeh Heredia (C11788) assaulted the officer at approximately 11:03 a.m. Inmate Heredia punched and choked the officer. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Heredia will receive a disciplinary report for this assault.
MILTON, Fla. – On May 11, an inmate assaulted a correctional officer at Santa Rosa Correctional Institution.
Inmate Eric Coleman (530148) assaulted the officer at approximately 12:27 p.m. Inmate Coleman punched the officer in the arm. Staff responded appropriately, and the inmate was subdued.

FLORIDA CITY, Fla. – On May 12, an inmate assaulted a correctional officer at Dade Correctional Institution.
Inmate Phillip Harris (140000) assaulted the officer at approximately 5:15 p.m. Inmate Harris punched the struck the officer in the face. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Harris will receive a disciplinary report for this assault.
MONITCELLO, Fla. – On May 12, an inmate assaulted correctional officers at Jefferson Correctional Institution.
Inmate Virgil Darville (W36738) assaulted the officers at approximately 12:10 p.m. Inmate Darville struck two officers in the face. Staff responded appropriately, and the inmate was subdued.
Inmate Darville will receive a disciplinary report for this assault.
MILTON, Fla. – On May 14, an inmate assaulted a correctional officer at Santa Rosa Correctional Institution.
Inmate Steven McDonald (195949) assaulted the officer at approximately 9:34 a.m. Inmate McDonald stabbed the officer in the shoulder with a makeshift weapon. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate McDonald will receive a disciplinary report for this assault.
CLERMONT, Fla. – On May 14, an inmate assaulted a correctional officer at Lake Correctional Institution.
Inmate Travis Tondee (Q12221) assaulted the officer at approximately 11:03 a.m. Inmate Tondee struck the officer in the face. Staff responded appropriately, and the inmate was subdued.
