FDC Assault Advisories | April 17 – April 23, 2018
This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty.
PERRY, Fla. – On April 17, an inmate assaulted a correctional officer at Taylor Correctional Institution.
Inmate James McCloud (091408) assaulted the officer at approximately 9:50 a.m. Inmate McCloud struck the officer in the chest with his head. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate McCloud will receive a disciplinary report for this assault.
MIAMI, Fla. – On April 17, an inmate assaulted a correctional officer at Everglades Correctional Institution.
Inmate Chad Mroczkowski (B15399) assaulted the officer at approximately 1:45 p.m. Inmate Mroczkowski struck the officer in the shoulder. Staff responded appropriately, and the inmate was subdued.
Inmate Mroczkowski will receive a disciplinary report for this assault.
DE FUNIAK SPRINGS, Fla. – On April 18, an inmate assaulted a correctional officer at Walton Correctional Institution.
Inmate Mark Jones (V50607) assaulted the officer at approximately 2:50 p.m. Inmate Jones struck the officer in the face. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Jones will receive a disciplinary report for this assault.
MADISON, Fla. – On April 18, an inmate assaulted a correctional officer at Madison Correctional Institution.
Inmate Willie Bolden (116251) assaulted the officer at approximately 3:15 p.m. Inmate Bolden pushed the officer from behind. Staff responded appropriately, and the inmate was subdued.
Inmate Bolden will receive a disciplinary report for this assault.
MADISON, Fla. – On April 19, an inmate assaulted a correctional officer at Madison Work Camp.
Inmate Albert Singletary (109575) assaulted the officer at approximately 10:25 p.m. Inmate Singletary kicked the officer. Staff responded appropriately, and the inmate was subdued.
Inmate Singletary will receive a disciplinary report for this assault.
BUSHNELL, Fla. – On April 18, an inmate assaulted a correctional officer at Sumter Work Camp.
Inmate Levar Dale (X51066) assaulted the officer at approximately 9:50 p.m. Inmate Dale struck the officer with his knee. Staff responded appropriately, and the inmate was subdued.
Inmate Dale will receive a disciplinary report for this assault.
LAKE CITY, Fla. – On April 20, an inmate assaulted a correctional officer at Columbia Annex.
Inmate Marty Marty (C10471) assaulted the officer at approximately 12:20 p.m. Inmate Marty struck the officer in the face. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Marty will receive a disciplinary report for this assault.
LAKE CITY, Fla. – On April 22, an inmate assaulted a correctional officer at Columbia Correctional Institution.
Inmate Aubrey Wormington (P14127) assaulted the officer at approximately 1:20 p.m. Inmate Wormington head-butted the officer in the face. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Wormington will receive a disciplinary report for this assault.
CHIPLEY, Fla. – On April 22, an inmate assaulted correctional officers at Northwest Florida Reception Center.
Inmate Kevin Flagg (J24004) assaulted the officer at approximately 8:09 p.m. Inmate Flagg stabbed the officer’s arm with a homemade weapon. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Flagg will receive a disciplinary report for this assault.
RAIFORD, Fla. – On April 22, an inmate assaulted a correctional officer at Florida State Prison.
Inmate Avery Hawthorne (H41307) assaulted the officer at approximately 8:58 p.m. Inmate Hawthorne kicked the officer in the face. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Hawthorne will receive a disciplinary report for this assault.
LAKE BUTLER, Fla. – On April 22, an inmate assaulted a correctional officer at Reception and Medical Center.
Inmate Michael Morris (915085) assaulted the officer at approximately 1:16 p.m. Inmate Morris struck the officer in the head and kicked the officer in the chest. Staff responded appropriately, and the inmate was subdued.
Inmate Morris will receive a disciplinary report for this assault.
LIVE OAK, Fla. – On April 22, an inmate assaulted a correctional officer at Suwannee Correctional Institution.
Inmate Kelvin Bradley (B12210) assaulted the officer at approximately 8:58 p.m. Inmate Bradley grabbed the officer and forced the officer to the ground. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Bradley will receive a disciplinary report for this assault.
BLOUNTSTOWN, Fla. – On April 23, an inmate assaulted a correctional officer at Calhoun Correctional Institution.
Inmate Rogerio Cavazos (W31477) assaulted the officer at approximately 12:52 a.m. Inmate Cavazos elbowed and punched the officer in the head. Staff responded appropriately, and the inmate was subdued.
Inmate Cavazos will receive a disciplinary report for this assault.
PUNTA GORDA, Fla. – On April 23, an inmate assaulted a correctional officer at Charlotte Correctional Institution.
Inmate Freddie Patterson (J01968) assaulted the officer at approximately 11:07 a.m. Inmate Patterson grabbed the officer and forced the officer to the ground. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Patterson will receive a disciplinary report for this assault.
MILTON, Fla. – On April 23, an inmate assaulted a correctional officer at Santa Rosa Correctional Institution.
Inmate Travis Wilson (B01216) assaulted the officer at approximately 2:26 p.m. Inmate Wilson head-butted the officer in the face. Staff responded appropriately, and the inmate was subdued.
Medical staff examined the officer and noted injuries. Inmate Wilson will receive a disciplinary report for this assault.
